Mounjaro

Mounjaro is a subcutaneous weight loss injection pen containing Tirzepatide as its active ingredient. It belongs to the Dual GIP/GLP-1 Receptor Agonist drug class — the first and only medication acting through two receptor types simultaneously. Manufactured by Eli Lilly.

Tirzepatide works by activating both GIP and GLP-1 receptors simultaneously, producing multiple effects:

• Reduces appetite — Acts on hunger control centres in the brain

Type 2 Diabetes: Mounjaro is approved for treating type 2 diabetes, providing excellent blood sugar control with greater HbA1c reduction than many other diabetes medications.

Weight Management / Obesity: For individuals with BMI ≥30 (obesity) or BMI ≥27 with comorbidities such as hypertension, dyslipidaemia, diabetes, or obstructive sleep apnoea.

Mounjaro's key advantage: The SURMOUNT-1 study showed Mounjaro 15 mg achieved an average weight loss of 22.5% of body weight (approximately 24 kg for a 107 kg person) — the highest weight loss achieved by any currently available injectable weight loss medication.

How to inject: Subcutaneous injection once weekly on the same day each week. Can be injected in the abdomen, thigh, or upper arm. Rotate injection sites each time.

Dose titration:

• Start at 2.5 mg/week for 4 weeks (starting dose, not therapeutic dose)
• Increase to 5 mg/week
• Increase every 4 weeks based on response: 7.5 mg → 10 mg → 12.5 mg → maximum 15 mg/week

Important: Must titrate gradually — do not skip doses as this increases GI side effects. Can be injected any day of the week, independent of meals.

If you miss a dose: Inject as soon as you remember. If more than 4 days late, skip and inject on the next scheduled day.

Common side effects:

• Nausea — most common, especially when starting and during dose increases; usually improves with continued use
• Vomiting
• Diarrhoea
• Constipation
• Abdominal pain, bloating
• Decreased appetite (part of the weight loss mechanism)

Serious side effects (stop taking and seek medical attention):

• Pancreatitis — Severe abdominal pain radiating to the back with severe nausea/vomiting
• Gastroparesis — Stomach paralysis with food not moving through
• Severe allergic reaction — Swelling of face, tongue, throat, difficulty breathing
• Gallbladder problems
• Hypoglycaemia — When used with other diabetes medications

Do not use:

• Personal or family history of Medullary Thyroid Carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
• History of pancreatitis
• Pregnant or planning pregnancy — must stop at least 2 months before conception
• Allergy to Tirzepatide

Use with caution:

• Kidney disease — risk of dehydration from vomiting/diarrhoea
• Gastroparesis or severe gastrointestinal conditions
• Concurrent use of other diabetes medications — monitor for hypoglycaemia
• Breastfeeding — insufficient safety data
• History of psychiatric disorders or suicidal ideation — monitoring required

Mounjaro vs. Wegovy (Semaglutide): Both are once-weekly weight loss injections. Mounjaro acts through dual receptors (GIP+GLP-1), while Wegovy acts through GLP-1 only. Studies show Mounjaro achieves greater weight loss (22.5% vs 15–17%), though it may be more expensive.

Mounjaro vs. Ozempic (Semaglutide): Ozempic contains the same active ingredient as Wegovy (Semaglutide) but is primarily approved for diabetes at a lower maximum dose. It achieves less weight loss than Mounjaro's dual-receptor mechanism.

Tirzepatide has a half-life of approximately 5 days, allowing once-weekly dosing. Peak blood levels are reached within 8–72 hours of injection.

For weight loss:

• Initial results noticeable within 4–8 weeks
• Weight loss continues throughout 72 weeks (per SURMOUNT-1 study)
• Maximum weight loss typically achieved at 60–72 weeks

For blood sugar control: HbA1c reduction begins within 4–8 weeks and continues up to 40 weeks.

Weight loss requires time and patience, combined with dietary changes and exercise.

Medications requiring caution:

• Sulfonylureas or Insulin — Combined with Mounjaro increases hypoglycaemia risk; doctor may need to reduce these medications
• Oral contraceptives — Mounjaro may reduce absorption due to delayed gastric emptying; consider alternative contraception or take 1 hour before Mounjaro injection
• Medications requiring rapid absorption — Mounjaro may reduce absorption of some medications

Safe combinations: Metformin can be safely used with Mounjaro and is commonly combined in diabetic patients.
