A combination tablet made by Ajanta Pharma (India). Each tablet holds Sildenafil (as citrate) 100 mg and Dapoxetine (as hydrochloride) 60 mg, for men who have both erectile dysfunction (ED) and premature ejaculation (PE).

Kamagra is a brand name used on several Sildenafil products, such as jelly, chewable and effervescent tablets. This tablet adds Dapoxetine, so it differs from a Kamagra that holds Sildenafil alone.

No label from the maker or a regulator was found for this combination itself. The safety information here therefore comes from the labels of Viagra (Sildenafil) and Priligy (Dapoxetine), which are single-ingredient products.

It is used by men who have two problems at the same time.

• Erectile dysfunction (ED) — Sildenafil blocks the PDE5 enzyme, so penile smooth muscle relaxes and more blood flows in. It needs sexual stimulation and is not an aphrodisiac.
• Premature ejaculation (PE) — Dapoxetine is a short-acting SSRI, presumed to work by blocking serotonin reuptake in nerve cells. Its label says to use it only as needed before activity, not every day.

The Dapoxetine label limits its use to men who have been diagnosed with premature ejaculation.

• Dose — 1 tablet, about 1-3 hours before sexual activity (the Dapoxetine window; the Sildenafil label says about 1 hour, range 30 minutes to 4 hours).
• Frequency — no more than 1 tablet in 24 hours, per both labels. Only when activity is planned, not daily.

• Sildenafil — headache, flushing, indigestion, nasal congestion, dizziness, nausea and visual changes such as blue-tinged vision. The US label lists flushing in 28% on 100 mg versus 7% on placebo.
• Dapoxetine 60 mg — nausea 22.2%, dizziness 10.9%, headache 8.8%, diarrhoea 6.9%, fatigue 4.1%, insomnia 3.9%, rising with dose; fainting in 0.23%.

Get medical help at once for an erection over 4 hours, sudden loss of vision or hearing, chest pain, fainting, an irregular heartbeat, or fever, agitation, stiff muscles and a rapid pulse after mixing with other medicines.

Do not use if (per the single-ingredient labels)

• Heart and blood pressure — heart failure, conduction problems, severe coronary or valve disease, past fainting, unstable angina, blood pressure below 90/50, or a recent stroke or heart attack.
• Liver — severe impairment (Sildenafil) or moderate and above (Dapoxetine).
• Eyes and mind — loss of vision in one eye from NAION, hereditary retinal disease, or a history of mania or severe depression.

Key caution: the Dapoxetine label says not to use it with PDE5 inhibitors, as blood pressure may fall on standing, and using both together has not been shown effective or safe. A single-dose study found Sildenafil 100 mg raised Dapoxetine 60 mg exposure only slightly (AUC +22%, Cmax +4%). If light-headed on standing, lie down or sit head between knees, and do not drive.

Ask a doctor first with uncontrolled epilepsy, angle-closure glaucoma or severe kidney impairment.

In short: with only one problem, a single-ingredient product such as Priligy or Viagra fits more closely. With both problems and a preference for a tablet, choose this one; prefer a jelly and the first two apply. Ask a pharmacist before using any.

Tell a doctor or pharmacist about every medicine and herb you take. The labels warn about these groups.

• Nitrates (chest-pain tablets, amyl nitrite or poppers) and riociguat — never combine; blood pressure can drop dangerously.
• Alpha-blockers (such as prostate medicines) — blood pressure may drop; it should be stable first.
• Serotonin-raising medicines such as SSRIs, SNRIs, tricyclics, tramadol, triptans, linezolid, lithium, L-tryptophan, St John's wort and MAOIs (14 days before, 7 after) — do not combine; serotonin syndrome can occur.
• CYP3A4 inhibitors such as ketoconazole, itraconazole and ritonavir — do not combine. With erythromycin, clarithromycin, fluconazole, verapamil or diltiazem Dapoxetine is capped at 30 mg, which this tablet cannot meet.
• Blood thinners, aspirin and NSAIDs — higher bleeding risk, so use with care. Avoid recreational drugs and sedatives.

The figures below come from the single-ingredient labels. No duration figure for the combination tablet was found from a regulator.

• Sildenafil — peak blood levels within 30-120 minutes (median 60) fasted, half-life 3-5 hours. A US label study showed response up to 4 hours, smaller than at 2 hours.
• Dapoxetine — peak about 1-2 hours after the dose, an initial half-life of roughly 1.5 hours, below 5% of peak by 24 hours, and a terminal half-life of about 19 hours.

So take it ahead of time, not at the start of activity; Sildenafil also needs sexual stimulation to work.
