Product Description

Herpes medication is essential for those infected with the Herpes Simplex Virus (HSV), which causes cold sores, genital herpes, and shingles. Vilerm is an antiviral (nucleoside analogue) herpes treatment containing Acyclovir 200/400/800 mg, manufactured by Siam Bheasach, a Thai pharmaceutical company.

📊 Key Information

  • Antiviral herpes medications work by inhibiting viral DNA polymerase, preventing the virus from replicating. However, they cannot eliminate dormant virus, so herpes cannot be completely cured
  • Genital Herpes — a common STI caused by HSV-2 (sometimes HSV-1)
  • Acyclovir reduces symptom severity, shortens healing time, and reduces recurrence frequency when used as suppressive therapy

🏭 Vilerm is manufactured by Siam Bheasach, a Thai pharmaceutical company. Thai FDA registered as a quality-equivalent generic of Acyclovir at an affordable price.

⚠️ When taking Acyclovir, it is crucial to drink plenty of water — at least 2–3 litres per day — to prevent drug crystallisation in the kidneys.

About Vilerm

What is Vilerm?

Vilerm is an oral herpes medication containing Acyclovir as its active ingredient. It belongs to the Antiviral (nucleoside analogue) drug class, manufactured by Siam Bheasach, a Thai pharmaceutical company.

Acyclovir works by inhibiting the viral DNA polymerase of Herpes viruses, preventing the virus from replicating its DNA, thus stopping viral reproduction and reducing the active viral load.

However, Acyclovir cannot eliminate dormant virus hiding in nerve ganglia, meaning herpes cannot be completely cured. The medication helps reduce severity, shorten healing time, and decrease transmission.

What is Vilerm Used For?

1. Cold Sores (Herpes Labialis): Caused by HSV-1, presenting as blisters on the lips. Acyclovir reduces severity and shortens healing time.

2. Genital Herpes: Caused by HSV-2 (or HSV-1), presenting as painful blisters on the genitals. Used for first-episode treatment, recurrence treatment, and suppressive therapy to prevent outbreaks.

3. Shingles (Herpes Zoster): Caused by Varicella-Zoster Virus (VZV) reactivating after chickenpox. Acyclovir reduces severity and lowers the risk of postherpetic neuralgia.

4. Chickenpox (Varicella): Used in severe cases or immunocompromised patients.

How to Take Vilerm / Dosage

First Episode: Take 200 mg 5 times daily (every 4 hours while awake) for 7–10 days.

Recurrent Episode: Take 200 mg 5 times daily for 5 days, or 800 mg 3 times daily for 2 days (short-course regimen). Start immediately at the first sign of symptoms (prodrome) such as itching, burning, or tingling.

Shingles: Take 800 mg 5 times daily (every 4 hours while awake) for 7 days.

Suppressive Therapy: Take 400 mg twice daily on an ongoing basis.

Very important: Drink plenty of water — at least 2–3 litres per day throughout treatment to prevent drug crystallisation in the kidneys and reduce the risk of kidney damage.

Side Effects of Vilerm

Common side effects:

  • Nausea, vomiting
  • Diarrhoea, abdominal pain
  • Headache
  • Dizziness, fatigue

Serious side effects (stop and seek medical attention):

  • Kidney failure / damage — caused by drug crystallisation in renal tubules, more likely with inadequate hydration. Drink plenty of water
  • Neurological symptoms — confusion, hallucinations, tremor, seizures (more common in renal impairment)
  • Severe allergic reaction — rash, hives, facial swelling, difficulty breathing
  • TTP/HUS — very rare, mainly in immunocompromised patients

Contraindications & Precautions

Do not use:

  • Allergy to Acyclovir or Valacyclovir (a prodrug of Acyclovir)

Use with caution:

  • Kidney disease — dose adjustment required based on creatinine clearance, as the drug is primarily renally excreted
  • Adequate hydration — at least 2–3 litres/day to prevent kidney damage
  • Elderly patients — reduced kidney function with age may require dose reduction
  • Pregnancy and breastfeeding — can be used when necessary (Category B) but consult your doctor
  • Immunocompromised patients — resistance may develop; close monitoring required

How Does Vilerm Compare to Other Herpes Medications?

Vilerm (Acyclovir) vs. Valacyclovir: Valacyclovir is a prodrug of Acyclovir — it converts to Acyclovir in the body. Its advantage is better absorption (54% vs 15–30% bioavailability), requiring fewer daily doses (2–3 times vs 5 times), making it more convenient. However, it costs more.

Vilerm (Acyclovir) vs. Famciclovir: Famciclovir is a prodrug of Penciclovir with good bioavailability and fewer daily doses. Efficacy is comparable to Valacyclovir, but it is significantly more expensive and less available in Thailand.

Advantages of Vilerm: Affordable price, longest history of clinical use, excellent safety profile, most extensive research data. Ideal for those seeking a cost-effective option.

Can Vilerm Be Taken with Other Medications?

Use with caution:

  • Probenecid — reduces renal excretion of Acyclovir, increasing blood levels
  • Nephrotoxic drugs — such as Aminoglycosides, high-dose NSAIDs; may increase kidney damage risk
  • Mycophenolate Mofetil — Acyclovir may increase blood levels of both drugs

Safe to combine:

  • General painkillers such as Paracetamol
  • Oral contraceptives
  • Vitamins and supplements

Most important: Drink plenty of water throughout treatment.

Frequently Asked Questions

No. Herpes is caused by the Herpes Simplex Virus, which remains dormant in nerve ganglia for life once infected. Acyclovir inhibits active viral replication, reducing symptoms, shortening healing time, and decreasing transmission, but cannot eliminate the dormant virus.

It depends on the indication. First episode/recurrence: 200 mg 5 times daily. Shingles: 800 mg 5 times daily. Suppressive therapy: 400 mg twice daily. Doses should be spaced every 4 hours while awake (skip the overnight dose). The frequent dosing is a key limitation of Acyclovir compared to Valacyclovir.

Acyclovir has limited water solubility. When excreted through the kidneys, it can crystallise in the renal tubules, causing obstruction and kidney damage. Drinking plenty of water (2–3 litres/day) dilutes the drug in urine and prevents crystallisation. This is the most important safety measure when using Acyclovir.

Valacyclovir (a prodrug of Acyclovir) has better absorption and requires fewer daily doses (2–3 vs 5), making it more convenient. Acyclovir is much cheaper, has the longest clinical use history, and the most safety data. Treatment efficacy is equivalent. The choice depends on budget and dosing convenience.

Yes. This is called suppressive therapy — taking Acyclovir 400 mg twice daily on an ongoing basis. It reduces recurrence frequency by up to 70–80% and also reduces transmission to sexual partners. Recommended for those with frequent outbreaks (more than 6 per year). Your doctor will reassess every 6–12 months.

Herpes is caused by HSV-1 or HSV-2, typically presenting as blisters on the lips or genitals with frequent recurrence. Shingles is caused by Varicella-Zoster Virus (VZV, same virus as chickenpox), presenting as a painful rash along a nerve pathway, usually occurring only once. Both are treated with Acyclovir, but shingles requires a higher dose (800 mg vs 200 mg).
