What is Cephalexin?

Cephalexin is a first-generation cephalosporin antibiotic that works by stopping bacteria from building their cell walls. Doctors commonly prescribe this 500 mg capsule for bacterial skin and soft-tissue infections and for urinary tract infections. It should be taken exactly as directed by a doctor or pharmacist for the full course to help prevent antibiotic resistance.

Product Description

A beta-lactam antibiotic, Cephalexin is classified as a first-generation cephalosporin. It works by binding to penicillin-binding proteins on the bacterial cell wall, blocking the final step of wall synthesis so the bacteria burst and die. Doctors reach for it when a skin or urinary tract infection is likely caused by susceptible bacteria, compared with Dicloxmed (dicloxacillin), which targets penicillinase-producing strains more specifically.

📊 Key facts:

  • Typical adult dose is 250-500 mg every 6-12 hours for 7-14 days depending on infection severity
  • Peak blood level is reached within about 1 hour after a dose, with a half-life of roughly 1-2 hours in people with normal kidney function
  • People with a history of penicillin allergy may cross-react with cephalosporins in up to about 10% of cases

🏭 Supplied as 500 mg capsules, 10 per strip, sized for short prescribed courses

⚠️ Always finish the full course even if symptoms improve early, to help prevent antibiotic resistance

About Cephalexin

What is Cephalexin?

Cephalexin is a first-generation cephalosporin, part of the beta-lactam antibiotic family alongside penicillins. It works by binding to penicillin-binding proteins on the bacterial cell wall, blocking the final cross-linking step of peptidoglycan synthesis (transpeptidation). This weakens the cell wall until the bacterium bursts and dies (autolysis). It mainly covers gram-positive bacteria, with some activity against certain gram-negative organisms found in urinary tract infections. It is sold as an oral capsule that is well absorbed from the digestive tract.

What does Cephalexin treat?

Doctors prescribe Cephalexin for susceptible gram-positive bacterial infections, including skin and soft-tissue infections (such as those caused by Staphylococcus aureus and Streptococcus pyogenes) and urinary tract infections. It is also used for some respiratory infections such as pneumonia and bronchitis, when the suspected bacteria are likely to respond to this class of antibiotic. It is often an early choice for outpatient treatment because of its long history of use and well-established safety profile, though the most suitable antibiotic for each case is always a clinical decision made by the treating doctor.

How to take Cephalexin / Dosage

The standard adult label dose is 250 mg every 6 hours, or 500 mg every 12 hours, for 7-14 days depending on how severe the infection is. NHS guidance describes 500 mg two to three times a day for at least 10 days for some conditions. For more severe infections, a doctor may raise the total daily dose to as much as 4 grams, split across several doses. It can be taken with or without food. Try to take each dose at the same times every day to keep blood levels steady, and continue for the full number of days prescribed even after symptoms improve.

Side effects of Cephalexin

Common side effects include mild diarrhea, nausea, stomach pain, heartburn, headache, dizziness, joint pain, and genital or rectal itching. These are usually not serious and settle as the course continues, but tell a doctor if they become severe or do not go away. Serious allergic symptoms that need immediate medical attention include hives, itching, swelling of the face, throat, tongue, lips, or eyes, difficulty breathing or swallowing, and wheezing. Stop the medicine and contact a doctor or pharmacist right away if any of these occur.

Contraindications / Precautions

Always tell a doctor or pharmacist before starting Cephalexin if there is a history of allergy to cephalosporins or penicillins, since cross-reactivity can occur in up to about 10% of people with a penicillin allergy history. This includes any other medicines you take, supplements, and kidney conditions, since the kidneys are the main route this drug leaves the body. In people with significantly reduced kidney function, its half-life can stretch to roughly 22 hours, so a doctor may need to lower the dose or space doses further apart based on kidney function.

Cephalexin vs. other antibiotics

Augmentin (Amoxicillin + Clavulanic acid) — broader coverage against penicillinase-resistant bacteria, taken just twice daily after meals, chosen when resistance is suspected. Dicloxmed (Dicloxacillin) — targets penicillin-resistant Staphylococcus specifically, often used for abscesses or more severe skin infections than typical cases.
Simply put: Cephalexin suits general, non-resistant skin and urinary tract infections and can be taken with or without food, while Augmentin and Dicloxmed are reached for when resistant bacteria are more likely.

Can Cephalexin be taken with other medicines?

Tell a doctor or pharmacist before combining Cephalexin with probenecid (gout medicine), metformin (diabetes medicine), or diuretics such as furosemide, since these may affect blood drug levels or kidney function. Probenecid works by reducing how quickly the kidneys clear this type of antibiotic, which can push blood levels higher than usual. Always share a full list of medicines being taken, including any other medicines you take, before starting treatment.

How fast does Cephalexin work?

After a dose, blood levels of Cephalexin peak within about 1 hour, with a half-life of roughly 1-2 hours in people with normal kidney function. This is why it needs to be taken several times a day exactly as prescribed, to keep drug levels high enough to kill bacteria throughout the course. Most of the drug is cleared through the kidneys still in its active form, which is part of why it works well for urinary tract infections. Symptoms usually start improving within a few days, but the full course should still be completed.

Frequently Asked Questions

It can be used in children at a doctor's discretion, with the dose calculated by body weight for each child. Do not split an adult capsule to give to a child without first checking with a doctor or pharmacist.

Always tell a doctor if pregnant, planning pregnancy, or breastfeeding, so the doctor can assess whether this medicine is necessary and appropriate on an individual basis before starting it.

Take it as soon as remembered. If it is almost time for the next dose, skip the missed one and continue as normal. Never double up, and always keep at least a 4-hour gap between doses.

Leftover capsules should not be saved for self-treatment later or shared with someone else, since each course is prescribed for a specific diagnosis and duration decided by a doctor for that individual.

Mild diarrhea is common and usually does not require stopping on your own, but if stools become very watery or bloody, with severe stomach pain or fever, stop the medicine and see a doctor right away.

Go back to see a doctor for reassessment. Further tests or a different antibiotic may be needed, since the bacteria causing the infection may not be sensitive to this medicine or another cause may be involved.

Do not flush unused or expired capsules down the toilet or drain. Return them to a pharmacy or hospital medicine take-back point, or seal them well before disposing with regular household waste as a pharmacist advises, to avoid environmental contamination or misuse.

Do not take them at the same time, since zinc may reduce how much of the antibiotic the body absorbs, making it work less well. Leave a gap of at least 3 hours before and after taking zinc supplements.

Both are beta-lactam antibiotics, but Cephalexin is a cephalosporin while Amoxicillin is a penicillin. A doctor chooses between them based on the suspected bacteria and each person's drug allergy history.
