Paracetamol

Paracetamol (known as acetaminophen in the US) is a non-opioid analgesic and antipyretic — the most widely used pain and fever medicine in the world. It acts on the central nervous system, reducing pain signaling and resetting the body's temperature set-point in the hypothalamus.

Unlike NSAIDs, it has no anti-inflammatory effect and is gentler on the stomach, making it a first choice for fever, headache, body aches, menstrual cramps, and mild toothache in both children and adults when dosed correctly. It has a strong safety profile and doesn't cause drowsiness, unlike some other painkillers.

Most adults take 500-1,000 mg (1-2 tablets of 500 mg) every 4-6 hours, up to 4 times a day, never exceeding 4 grams total per day.

If pain persists beyond 5 days, or fever beyond 3 days, stop and see a doctor or pharmacist rather than increasing the dose or shortening the interval — doing so raises liver risk without added benefit.

If a dose is missed and it's close to the next one, skip it rather than doubling up. Taking it for more than 5-7 days without medical advice isn't recommended, since it can mask symptoms needing proper diagnosis.

The pharmacist rule for dosing children is 10-15 mg/kg per dose every 4-6 hours, up to 5 doses a day. For example, a 15 kg child gets 150-225 mg per dose — roughly 3-4.5 ml of 250mg/5ml syrup.

Never give more than 5 doses in 24 hours, and always use the dosing cup that comes with the product — a kitchen spoon isn't accurate. For infants under 3 months with fever, see a doctor before giving any medicine.

Tablet-form paracetamol typically starts working within 30-60 minutes, peaks at 1-2 hours, and lasts 4-6 hours. Children's liquid syrup tends to act slightly faster since it absorbs more readily.

Taking it on an empty stomach may speed onset slightly, but if your stomach is sensitive, taking it with water or a light snack works too — unlike NSAIDs, it doesn't need to be taken with food. If there's no improvement after 2 hours, consider other causes and check with a pharmacist.

Paracetamol reduces fever and pain well but has no anti-inflammatory effect, making it suited to fever, headaches, and general aches. NSAIDs (like ibuprofen) add anti-inflammatory action, better suited to pain with swelling — joint pain or muscle inflammation.

A doctor may sometimes suggest alternating paracetamol with an NSAID for a stubborn fever, but only under medical guidance — don't combine both yourself.

Paracetamol overdose is a leading cause of acute liver failure worldwide, and the most dangerous scenario is accidental double-dosing — many cold remedies, combination painkillers, and fever medicines already contain paracetamol.

The anti-duplication rule: before taking any new medicine, check the label for "Paracetamol" or "Acetaminophen" and add up total daily intake across every product to stay under 4 grams. Warning signs of toxicity include nausea, vomiting, upper-right abdominal pain, and jaundice — seek medical care immediately.

If overdose is suspected, go to a hospital immediately even without symptoms — liver damage may not appear within the first 24 hours, and antidote treatment works best given early.
