About Bestatin
What is Bestatin?
Bestatin is a statin cholesterol-lowering medication containing Simvastatin as its active ingredient. It belongs to the HMG-CoA Reductase Inhibitor class. The drug works by blocking the enzyme HMG-CoA Reductase, which plays a central role in cholesterol production in the liver.
When the liver's cholesterol production decreases, the body responds by increasing the number of LDL Receptors on liver cell surfaces, pulling more LDL (bad cholesterol) out of the bloodstream. This results in a significant reduction of blood LDL levels.
Simvastatin is a prodrug — it must be converted to its active form in the liver before it can work. It is metabolised via the CYP3A4 enzyme, which is why many drug interactions exist.
Bestatin is classified as a dangerous drug by the Thai FDA and must be used under the guidance of a physician or pharmacist.
What is Bestatin used for?
Bestatin is a cholesterol-lowering medication used for treating hyperlipidemia/hypercholesterolemia by reducing Total Cholesterol, LDL-C, and Triglycerides while increasing HDL-C (good cholesterol); preventing cardiovascular disease in patients with a history of coronary heart disease (secondary prevention) or those at high risk such as diabetics, hypertensive patients, and smokers; reducing the risk of stroke; and decreasing the need for coronary revascularisation procedures such as stenting or bypass surgery.
Cholesterol-lowering medication must be used alongside lifestyle changes including dietary control, regular exercise, weight management, and smoking cessation.
How to Take Bestatin / Dosage
Adults: Take once daily in the evening or at bedtime. This timing is important because the body produces the most cholesterol at night — taking the medication during this period maximises its effectiveness. Can be taken with or without food. Take at the same time each day.
Treatment duration: Statin therapy is typically a long-term or lifelong commitment. Do not stop the medication when blood results improve, as cholesterol levels will rise again within weeks.
Children aged 10–17: May be used only for Familial Hypercholesterolemia under medical supervision.
Severe kidney impairment: Dose adjustment may be required. Consult your doctor.
Missed dose: Take it as soon as you remember. If it's nearly time for the next dose, skip the missed one. Never double the dose.
Side Effects of Bestatin
Common side effects:
- Headache, dizziness, insomnia
- Constipation, bloating, nausea
- Mild muscle pain (Myalgia)
Serious side effects (stop immediately and seek medical attention):
- Severe unexplained muscle pain or weakness — may indicate Myopathy or Rhabdomyolysis (muscle breakdown), a life-threatening condition
- Dark-coloured urine (cola-coloured) — a warning sign of Rhabdomyolysis
- Yellowing of skin or eyes (jaundice) — may indicate liver damage
- Unusual fatigue, loss of appetite, upper abdominal pain — signs of hepatitis
The risk of Rhabdomyolysis increases with high doses and when combined with drugs that inhibit the CYP3A4 enzyme.
Contraindications & Precautions
Do not use:
- Known allergy to Simvastatin or any other statin
- Active liver disease or unexplained persistent elevations in liver enzymes
- Pregnancy and breastfeeding — absolutely contraindicated. Cholesterol is essential for foetal development; statins may cause birth defects
- Concurrent use of: Itraconazole, Ketoconazole, Erythromycin, Clarithromycin, HIV Protease Inhibitors, Gemfibrozil, Cyclosporine, Danazol
Use with caution:
- Heavy alcohol consumption — increases liver damage risk
- Elderly — higher risk of muscle-related side effects
- Uncontrolled hypothyroidism — increases Myopathy risk
- Avoid large quantities of grapefruit juice — inhibits CYP3A4, dangerously increasing drug levels
- Regular blood tests to monitor liver enzymes and lipid levels are recommended
How Does Bestatin Compare to Other Cholesterol-Lowering Medications?
Bestatin vs. Zimva (Simvastatin): Both contain the same active ingredient (Simvastatin) and offer equivalent therapeutic effectiveness. The difference is the manufacturer. Bestatin is made by Berlin Pharmaceutical, while Zimva is produced by a different manufacturer. Either can be used as recommended by your pharmacist. See all Simvastatin options in the Simvastatin collection.
Simvastatin vs. Atorvastatin: Both are statin cholesterol-lowering medications. Atorvastatin is a newer-generation statin with a longer half-life (can be taken at any time of day), greater LDL reduction at equivalent doses, and fewer CYP3A4-related drug interactions. Simvastatin has robust long-term safety data from the landmark 4S Study and is more affordable.
Simvastatin vs. Rosuvastatin: Rosuvastatin is the most potent statin for LDL reduction. It is not metabolised through CYP3A4, resulting in fewer drug interactions. However, it is more expensive. Simvastatin remains a suitable choice for those needing an effective and affordable cholesterol-lowering medication.
Can Bestatin Be Taken with Other Medications?
Simvastatin is metabolised via the CYP3A4 enzyme in the liver, creating numerous significant drug interactions.
Absolutely contraindicated combinations:
- Antifungals: Itraconazole, Ketoconazole, Posaconazole, Voriconazole
- Antibiotics: Erythromycin, Clarithromycin, Telithromycin
- HIV medications: Protease Inhibitors (e.g. Ritonavir, Nelfinavir)
- Gemfibrozil, Cyclosporine, Danazol
Dose-limiting combinations:
- Amiodarone, Verapamil, Diltiazem — may increase Myopathy risk
- Amlodipine — Simvastatin dose should not exceed specified limits
Use with monitoring:
- Warfarin — Simvastatin may enhance anticoagulant effect; INR monitoring required
- Digoxin — Digoxin levels may increase slightly
Always inform your pharmacist about all medications you are taking, including supplements and herbal products.
How Long Does Bestatin Take to Work?
Simvastatin is rapidly absorbed after oral administration. As a prodrug, it requires conversion in the liver. Peak levels of the active metabolite occur approximately 1–2 hours after taking the dose. The half-life is approximately 2–3 hours, which is why evening dosing is required to coincide with peak cholesterol production overnight.
Cholesterol-lowering effects become measurable within 2–4 weeks of starting treatment and reach maximum effect at 4–6 weeks. Your doctor will then check blood tests to evaluate results and adjust the dose if needed.
Stopping the medication causes cholesterol levels to return to pre-treatment levels within a few weeks.

Frequently Asked Questions
Because the body produces the most cholesterol at night, and Simvastatin has a short half-life (approximately 2–3 hours). Taking the medication in the evening ensures peak drug levels coincide with the period of maximum cholesterol production, giving the best therapeutic effect.
Statin cholesterol-lowering medications are typically taken long-term, often for life. This maintains cholesterol levels at target goals and prevents cardiovascular complications. If you stop taking the medication, cholesterol levels will return to pre-treatment levels within weeks.
Both are statin medications. Simvastatin (Bestatin) is an older-generation statin with robust long-term safety data from the 4S Study (4,444 patients, 5.4 years). It requires evening dosing and is more affordable. Atorvastatin is newer, achieves greater LDL reduction at equivalent doses, and can be taken at any time of day.
Absolutely. Medication is only one part of treatment. Reducing saturated fats, trans fats, and fried foods, combined with regular exercise, weight management, and smoking cessation, will produce the best treatment outcomes.
Mild muscle pain is a common side effect and usually not serious. However, if you experience severe unexplained muscle pain, muscle weakness, or dark-coloured urine, stop the medication immediately and see a doctor. These may be signs of Rhabdomyolysis (muscle breakdown), which is a medical emergency.
Grapefruit juice inhibits the CYP3A4 enzyme in the liver — the enzyme responsible for breaking down Simvastatin. When this enzyme is blocked, drug levels in the blood rise significantly, greatly increasing the risk of serious side effects, particularly muscle damage. Avoid large amounts of grapefruit juice throughout your treatment.
