Norgesic

Norgesic is a muscle relaxant combined with an analgesic containing two active ingredients: Orphenadrine Citrate 35 mg and Paracetamol 450 mg. It is classified as a Muscle Relaxant + Analgesic Combination, manufactured by iNova/Organon.

Orphenadrine acts as an Anticholinergic at the muscle control centres in the brain, reducing muscle spasm. Combined with Paracetamol, which inhibits Prostaglandin in the central nervous system to reduce pain.

The combination of both drugs in a single tablet provides simultaneous relief of muscle spasm and pain, ideal for muscle pain accompanied by spasm.

Muscle pain: Norgesic treats muscle pain, back pain, neck pain, and muscle spasms caused by injury, overuse, or poor posture.

Key indications:

• Acute muscle pain
• Low back pain
• Neck pain, stiff neck
• Muscle spasm
• Soft tissue injury pain

Not for: Spasticity from brain or spinal cord conditions such as Multiple Sclerosis or Cerebral Palsy.

Dosage: Take 1–2 tablets, 3 times daily after meals or when needed. Swallow whole with water.

Duration: Use only as needed. Do not take continuously for extended periods without medical advice. For acute conditions, typically 1–2 weeks.

If you miss a dose: Skip the missed dose and take the next one as scheduled. Do not double the dose.

Important:

• Watch for duplicate Paracetamol from other medicines. Total Paracetamol from all sources must not exceed 4,000 mg/day
• Causes drowsiness — do not drive or operate machinery
• Do not drink alcohol while taking this medicine, as it increases drowsiness and liver toxicity risk

Common side effects:

• Drowsiness
• Dry mouth
• Blurred vision
• Constipation
• Nausea

Serious side effects:

• Paracetamol toxicity if overdosed (more than 4,000 mg/day) — causes liver damage
• Urinary retention
• Tachycardia (fast heartbeat)
• Confusion, especially in the elderly

When to stop and see a doctor:

• Allergic reactions such as rash, swelling, or difficulty breathing
• Inability to urinate
• Confusion or hallucinations
• Pain does not improve after 1 week of use

Do not use:

• Glaucoma patients
• Patients with spasticity from brain or spinal cord conditions (CNS Spasticity)
• Myasthenia Gravis patients
• Allergy to Orphenadrine or Paracetamol
• Severe liver disease

Use with caution:

• Do not drive or operate machinery while taking this medication
• Do not drink alcohol — increases sedation and risk of liver toxicity from Paracetamol
• Watch for duplicate Paracetamol — check ingredients of other medications
• Elderly patients may experience stronger Anticholinergic side effects
• Patients with enlarged prostate — risk of urinary retention

Norgesic vs. Mydocalm (Tolperisone): Mydocalm causes less drowsiness than Norgesic as it acts at the spinal cord level without significantly crossing the Blood-Brain Barrier. Better for those who need to drive or concentrate. However, Norgesic has the advantage of including Paracetamol for pain relief.

Norgesic vs. Myonal (Eperisone): Myonal also causes less drowsiness. It is a newer muscle relaxant with fewer side effects but does not include an analgesic — a separate pain reliever is needed if pain is present.

• Orphenadrine: Reaches full effect within 1–2 hours, with a half-life of approximately 14 hours for prolonged action

For acute conditions: Improvement is usually seen within 2–3 days. Chronic conditions may take longer.
