Postinor

Postinor is an Emergency Contraceptive containing Levonorgestrel 0.75 mg (2 tablets per pack). Manufactured by Gedeon Richter, Hungary.

Levonorgestrel is a synthetic Progestogen that provides emergency contraception through key mechanisms:

• Preventing or delaying ovulation — if ovulation has not yet occurred, the drug prevents the egg from being released
• Disrupting sperm transport — thickens cervical mucus, making it harder for sperm to reach the egg

Important: Postinor is not an abortion pill. If fertilisation and implantation have already occurred, the drug will not affect an existing pregnancy.

Postinor is WHO-approved and has been used worldwide for decades. It is safe when used as directed.

Use when:

• Unprotected sexual intercourse
• Condom broke or slipped off
• Missed regular contraceptive pills
• Miscalculated safe period
• Other emergency situations with risk of unintended pregnancy

Timing: Must be taken within 72 hours (3 days) after intercourse. The sooner, the more effective:

• Within 24 hours: 95% effective
• Within 24–48 hours: 85% effective
• Within 48–72 hours: 58% effective

Not for: Regular daily contraception. For ongoing contraception, use daily pills or other suitable methods.

Method 1 (Recommended): Take both tablets together (total Levonorgestrel 1.5 mg) as soon as possible after intercourse, within 72 hours.

Method 2: Take the first tablet as soon as possible, then the second tablet 12 hours later.

Recommendations:

• Can be taken at any point in the menstrual cycle, with or without food
• Swallow whole with water
• If vomiting occurs within 2 hours, take another dose
• The sooner you take it, the more effective it is

After taking:

• Use condoms or other contraception until your next period arrives normally
• Your period may arrive 1–2 weeks earlier or later than expected
• If your period is more than 3 weeks late, take a pregnancy test

Common side effects:

• Nausea — most common
• Lower abdominal pain
• Headache
• Fatigue
• Spotting (irregular bleeding)
• Period arriving earlier or later than expected
• Breast tenderness

Non-serious but noteworthy effects:

• Next period may be heavier or lighter than normal
• Irregular bleeding may occur 1–2 weeks after use

When to see a doctor:

• Severe vomiting within 2 hours of taking (need to re-dose)
• Period does not arrive within 3 weeks (may be pregnant)
• Severe lower abdominal pain (possible ectopic pregnancy)
• Allergic reactions such as rash, swelling, or difficulty breathing

Do not use:

• Allergy to Levonorgestrel or any component
• Known existing pregnancy (it will not cause abortion, but is unnecessary)

Use with caution:

• Should not be used as regular contraception — less effective than daily pills and may disrupt menstrual cycles
• Severe liver disease — consult a doctor before use
• History of ectopic pregnancy or fallopian tube inflammation — use with extra caution
• Does not protect against sexually transmitted infections (STIs)
• After using emergency contraception, start a regular contraceptive method promptly
• Body weight over 70 kg may reduce effectiveness — consult a pharmacist

Postinor vs. Madonna (Levonorgestrel 1.5 mg single tablet): Same active ingredient and total dose (1.5 mg). Postinor comes as 2 x 0.75 mg tablets (taken together or 12 hours apart), while Madonna is a single 1.5 mg tablet. Equal efficacy.

Postinor vs. EllaOne (Ulipristal Acetate): EllaOne contains Ulipristal Acetate 30 mg, effective up to 120 hours (5 days), with more consistent efficacy than Levonorgestrel at 72–120 hours. However, it is harder to find in Thailand and more expensive.

Postinor vs. daily contraceptive pills: Daily pills such as Cerazette are far more effective (99%+) when used correctly. Postinor is for emergency use only and should not replace daily contraception.

Postinor starts working immediately after ingestion. Levonorgestrel is rapidly absorbed, reaching peak blood levels within 1–2 hours.

Effectiveness by timing:

• Within 12 hours: Most effective, ~95% prevention
• 12–24 hours: ~95% prevention
• 24–48 hours: ~85% prevention
• 48–72 hours: ~58% prevention
• After 72 hours: Significantly reduced effectiveness — not recommended

Half-life: Levonorgestrel has a half-life of approximately 26 hours. The drug is eliminated from the body within 2–3 days.

Remember: The sooner you take it after intercourse, the better. Do not wait until the last minute.
