About Omeprazole GPO
What is Omeprazole GPO?
Omeprazole GPO is a stomach acid reducer in the Proton Pump Inhibitor (PPI) class containing Omeprazole 20 mg, manufactured by the Government Pharmaceutical Organization (GPO) of Thailand.
Omeprazole works by irreversibly inhibiting the Proton Pump (H+/K+ ATPase) on stomach cell walls, reducing acid secretion by up to 90% with continuous use.
Omeprazole GPO is a racemic mixture (combination of R-isomer and S-isomer), unlike Nexium which contains only the S-isomer (Esomeprazole). However, clinical efficacy differs only slightly, and Omeprazole GPO is significantly cheaper.
What is Omeprazole GPO used for?
Key indications:
- GERD (Gastroesophageal Reflux Disease) — heartburn, acid regurgitation
- Gastric Ulcer
- Duodenal Ulcer
- Zollinger-Ellison Syndrome
- Combined with antibiotics for H. pylori eradication
- Prevention of NSAID-induced ulcers
Omeprazole GPO has the same indications as Nexium but is a racemic mixture at a much lower price — ideal for those requiring long-term use who want to save costs.
How to Take Omeprazole GPO / Dosage
Dosage: Take Omeprazole 20 mg once daily, 30 minutes before breakfast. Swallow the capsule whole with water — do not chew or crush.
Duration:
- GERD: 4–8 weeks
- Gastric ulcer: 4–8 weeks
- Duodenal ulcer: 2–4 weeks
- H. pylori eradication: 7–14 days (with antibiotics)
If you miss a dose: Take it as soon as you remember. If close to the next dose, skip it. Do not double the dose.
Important:
- Must be taken 30 minutes before food for optimal effect
- Do not use continuously for more than 8 weeks without medical advice
- If symptoms persist, see a doctor for further investigation
Side Effects of Omeprazole GPO
Common side effects:
- Headache
- Nausea, diarrhoea
- Abdominal pain, bloating
- Constipation
Long-term side effects (over 1 year):
- Hypomagnesemia (low magnesium)
- Vitamin B12 deficiency
- Increased fracture risk
- Increased risk of C. difficile infection
- Fundic Gland Polyps in the stomach (benign)
When to see a doctor:

Contraindications & Precautions
Do not use:
- Allergy to Omeprazole or other PPIs
- Do not use with Rilpivirine (HIV antiviral)
- Do not use with Nelfinavir
Use with caution:
- Ensure no gastric malignancy before starting — PPIs may mask symptoms
- Severe liver impairment — dose reduction may be needed
- Concurrent use with Clopidogrel — Omeprazole may reduce Clopidogrel efficacy
- Caution with Methotrexate, Warfarin, Phenytoin interactions
- Pregnant or breastfeeding women — consult doctor first
How Does Omeprazole GPO Compare to Other Acid Reducers?
Omeprazole GPO vs. Nexium (Esomeprazole): Nexium contains only the S-isomer (Esomeprazole), which some studies suggest may be slightly better in certain cases. However, the clinical difference is small. Nexium costs significantly more. Omeprazole GPO is a highly cost-effective option for long-term use.
Omeprazole GPO vs. Famotidine (H2 Blocker): Omeprazole is a PPI, which suppresses acid much more potently than H2 Blockers. Suitable for severe symptoms or ulcers. Famotidine is better for mild symptoms or occasional use.
How Long Does Omeprazole GPO Take to Work?
Omeprazole starts reducing acid within 1 hour, but it takes 2–5 days to feel noticeable improvement as the drug needs to accumulate and inhibit enough Proton Pumps.
Results timeline:
- Day 1: Acid reduction begins but may not be noticeable
- 2–5 days: Heartburn significantly improves
- 2–4 weeks: Ulcers begin to heal
- 4–8 weeks: Full healing for most patients
If symptoms don't improve within 2 weeks, consult your doctor.

Frequently Asked Questions
Very similar. Omeprazole GPO is racemic Omeprazole while Nexium is Esomeprazole (the S-isomer of Omeprazole). Some studies suggest Esomeprazole may be slightly better, but the clinical difference is minimal. Omeprazole GPO is significantly cheaper — ideal for long-term use.
Yes. Take 30 minutes before meals (especially breakfast) because Omeprazole works best when Proton Pumps are active, which happens during eating. Taking it after meals significantly reduces effectiveness.
Long-term PPI use (over 1 year) may increase risk of magnesium deficiency, vitamin B12 deficiency, bone fractures, and C. difficile infection. Use the lowest effective dose and attempt to taper off when symptoms improve. Consult your doctor regularly.
Not recommended. Omeprazole inhibits CYP2C19, the enzyme that converts Clopidogrel to its active form, reducing its effectiveness. If a PPI is needed with Clopidogrel, switch to Pantoprazole instead.
Yes. Abruptly stopping PPIs can cause Rebound Acid Hypersecretion — the stomach temporarily produces more acid than normal. Therefore, taper the dose gradually before stopping, such as taking it every other day before discontinuing.
Omeprazole is Pregnancy Category C — use only when benefits outweigh risks. No clear evidence of harm, but always consult a doctor first. For acid reflux during pregnancy, doctors may recommend antacids or H2 blockers first.
