About Ritalin

What is Ritalin?

Ritalin 10mg is the trade name of a tablet holding Methylphenidate hydrochloride 10 mg. Methylphenidate is an immediate-release central nervous system stimulant (ATC code N06BA04). The product listed here is a box of 100 tablets, with Novartis named on the box.

The UK label says the effect is thought to come from blocking the reuptake of dopamine and norepinephrine into nerve cells. The US label says how it works in ADHD and narcolepsy is not known.

The leaflet differs by country: the US label is issued by Sandoz Inc, the UK label by InfectoPharm and the Canadian monograph by Novartis Pharmaceuticals Canada, and the tablet and pack differ by market too. Rely on the box and leaflet you receive.

What is Ritalin used for?

The uses listed on each country's label:

  • United Kingdom (SmPC) ADHD as part of a comprehensive treatment programme, in children and adolescents aged 6 and over and in adults, when remedial measures alone are not enough.
  • United States (DailyMed) ADHD in children aged 6 and over and in adults, and narcolepsy.
  • Canada (Product Monograph) ADHD in children aged 6 and over and in adults, and narcolepsy.

The English NICE guideline NG87 puts non-drug measures first when the impairment is mild, and asks for a heart and blood pressure check before starting.

Using it without ADHD or narcolepsy is not a use on any of these labels, and the UK label says not to use it for normal fatigue.

How to take Ritalin

Label doses differ by country. Every label says to start low, adjust slowly and use the lowest dose that works, as a decision for the care team.

  • Children aged 6 and over Start at 5 mg once or twice a day (UK) or twice a day (US), raised by 5-10 mg a week, up to 60 mg a day.
  • Adults The UK label starts at 10 mg twice a day, up to 80 mg a day. The US and Canadian labels average 20-30 mg a day in 2-3 doses, up to 60 mg.
  • Last dose The UK label says not within 4 hours of bedtime; the US and Canadian labels say before 6 p.m. if sleep is a problem.
  • With food The UK label allows it with or after a meal, which eases nausea. The US label tells adults to take it 30-45 minutes before meals.

The scored tablet can be halved (UK label). If a dose is missed, never double up. The labels say to stop if there is no improvement after one month of dose adjustment.

Side effects of Ritalin

Frequencies below come from the UK label (children, adolescents and adults). The US label has no frequency table.

  • Very common (over 1 in 10 people) decreased appetite, trouble sleeping, nervousness, headache, nausea, dry mouth.
  • Common anxiety, irritability, aggression, dizziness, palpitations, high blood pressure, stomach pain, weight loss in adults, and slower weight and height gain in children on long use.
  • Uncommon psychotic disorders, hallucinations, thoughts of self-harm, tics, blurred vision, chest pain.
  • Very rare seizures, heart attack, cardiac arrest, abnormal liver function.
  • Not known dependence.

The NHS says to call a doctor for mood or personality changes, a fast or irregular heartbeat, hallucinations or facial tics.

Contraindications and warnings

Boxed warning on the US label (DailyMed): the medicine has a high potential for abuse and misuse, which can lead to addiction. Misuse can cause overdose and death, especially at higher doses or by snorting or injection. In the US it is a Schedule II controlled substance. Store it securely.

  • Do not use if allergic to the drug, or taking a monoamine oxidase inhibitor (MAOI), see the interactions tab. The UK and Canadian labels add glaucoma, an overactive thyroid, phaeochromocytoma and some serious heart disorders; the UK label also adds severe depression, psychotic conditions and some brain blood vessel disorders.
  • Children under 6 should not use it.
  • Heart sudden death has been reported in people with serious heart disease. Palpitations, chest pain or fainting need a prompt heart check.
  • Mind it can worsen psychosis or trigger mania. About 0.1% had new psychotic or manic symptoms in short trials (US label).
  • Children need weight and height monitored.
  • A prolonged painful erection (priapism) needs urgent medical attention.
  • Raynaud's numb, cool or colour-changing fingers or toes.
  • Other epilepsy, glaucoma, tics, drowsiness (take care driving), and false positive amphetamine urine tests (UK label).

Can Ritalin be taken with other medicines?

Tell the care team about every medicine you take, including ones you bought yourself.

  • MAOIs must not be combined with it, or used within 14 days of stopping, because of the risk of a hypertensive crisis (US and UK labels).
  • Blood pressure medicines may work less well, so blood pressure needs monitoring.
  • Halogenated anaesthetics risk a sudden rise in blood pressure during surgery, so the UK label says not to use Ritalin on the day of surgery.
  • Coumarin blood thinners, anticonvulsants and some antidepressants may be cleared more slowly (UK label).
  • Risperidone and antipsychotics changing the dose of either can raise the risk of stiffness and tremor (US label).
  • Clonidine long-term safety of the combination has not been studied systematically.
  • Alcohol can worsen the side effects, and the UK label says to abstain during treatment.

How fast does Ritalin work?

The UK label says the effect starts within about 1 hour of a suitable dose and lasts up to 4 hours, so it is taken several times a day.

The tablet is absorbed quickly and almost completely. Blood levels peak at about 1-2 hours (UK) or about 2 hours (US and Canada) and vary a lot between people. The half-life is about 2 hours (UK label) or 2.1-2.4 hours (Canadian label).

The body breaks it down into ritalinic acid, which has little or no activity, and about 78-97% of a dose leaves in the urine within 48-96 hours. Food raised the peak level by 23% (UK label).

That is not a labelled use. Ritalin is listed for ADHD and narcolepsy (US and Canadian labels). The UK label says it should not be used for normal fatigue, and taking it without a listed condition carries the same heart, mental health and addiction risks.

Call a doctor or go to hospital straight away for chest pain, a fast or irregular heartbeat, thoughts of harming yourself or others, hallucinations, a seizure, stroke signs such as a drooping face or slurred speech, a prolonged painful erection, or a serious allergic reaction such as swelling of the lips or throat and difficulty breathing.

The UK label says not above 25°C, in the original package to protect from moisture, with a shelf life of 2 years. The US label says 20-25°C away from light, and the Canadian label 2-30°C away from humidity. Keep it out of reach of children and count the tablets from time to time, as the medicine can be misused.
