About Sumigran

What is Sumigran?

Sumigran is an oral migraine medication containing Sumatriptan as its active ingredient. It belongs to the Triptan (5-HT1B/1D Agonist) drug class and is a Thai generic.

Sumatriptan works by stimulating 5-HT1B/1D Serotonin receptors, producing two simultaneous effects: constricting dilated cranial blood vessels during a migraine and reducing neuropeptide release that causes inflammation and pain.

Sumatriptan was the first Triptan developed (1991) and remains the most widely used migraine medication globally. It acts rapidly, within 30 minutes to 1 hour.

What is Sumigran used for?

Acute migraine: Sumigran treats ongoing migraine attacks, both with and without aura (warning symptoms like visual disturbances). It relieves headache, nausea, photophobia, and phonophobia.

Important — Not preventive: Sumigran is taken only at the onset of a migraine attack. It is not for daily preventive use. If migraines occur frequently (more than 4 times/month), consult your doctor about preventive therapy.

Not for: Headaches from other causes such as tension headache, cluster headache, or hypertensive headache. A migraine diagnosis should be confirmed before starting.

How to Take Sumigran / Dosage

Starting dose: Take Sumatriptan 50 mg at the onset of migraine symptoms. Swallow whole with water.

If not improved: A second dose can be taken after 2 hours, but do not exceed 300 mg per day (for 50 mg = max 6 tablets/day, for 100 mg = max 3 tablets/day).

If 50 mg is insufficient: Your doctor may increase to 100 mg per dose.

Important advice:

  • Take as soon as migraine pain begins — do not wait until it becomes severe
  • If the first dose doesn't work for the same attack, do not take another for the same episode; wait until a new migraine occurs
  • Using triptans more than 10 days/month may cause Medication Overuse Headache (MOH)

Side Effects of Sumigran

Common side effects:

  • Chest/throat tightness (triptan sensation) — common, not dangerous, characteristic of Triptans
  • Flushing, feeling of warmth
  • Fatigue, drowsiness
  • Tingling in hands/feet (paraesthesia)
  • Nausea (may also be from the migraine itself)

Serious side effects (stop and seek medical attention):

  • Cardiac vasospasm — severe chest pain, significant chest tightness, arrhythmia
  • Serotonin Syndrome — agitation, muscle twitching, fever (especially when combined with SSRI/SNRI)
  • Severe allergic reaction — rash, swelling, difficulty breathing
  • Seizures

Note: Chest/throat tightness is a common triptan sensation unrelated to the heart. However, consult your doctor if severe.

Contraindications & Precautions

Do not use:

  • Ischaemic heart disease or coronary artery disease
  • History of stroke or TIA
  • Uncontrolled hypertension
  • Do not combine with MAOi or Ergotamine
  • Allergy to Sumatriptan

Use with caution:

  • Caution with SSRI/SNRI — risk of Serotonin Syndrome
  • Cardiovascular risk factors (smoking, diabetes, high cholesterol, obesity) — medical assessment recommended before starting
  • Epilepsy patients
  • Severe liver disease — dose reduction may be needed

How Does Sumigran Compare to Other Migraine Medications?

Sumigran vs. Tofago (Ergotamine): Tofago is an older Ergotamine-class medication. Sumigran (Sumatriptan) is a newer Triptan — more effective, more targeted, and with fewer side effects. Doctors now prefer Triptans over Ergotamine due to superior efficacy and safety.

Sumigran vs. Ibuprofen/Paracetamol: Common painkillers may help mild-to-moderate migraines, but for severe migraines, Sumatriptan is far more effective as it targets the underlying cause of migraine directly, not just pain relief.

How Long Does Sumigran Take to Work?

Sumatriptan is rapidly absorbed. Peak blood levels are reached within 2 hours after taking.

Duration of action:

  • Onset within 30 minutes to 1 hour
  • Most patients (60–70%) experience significant relief within 2 hours
  • Effects last approximately 4–6 hours

Headache recurrence: Approximately 30–40% of patients may experience headache recurrence within 24 hours. A second dose can be taken after 2 hours.

Important: The sooner you take it after migraine onset, the better it works. Don't wait until pain becomes severe.

Frequently Asked Questions

Sumigran (Sumatriptan) is a newer Triptan with higher efficacy and fewer side effects. Tofago (Ergotamine) is an older, less specific medication with more adverse effects. Doctors now recommend Triptans as the first-line treatment for acute migraines.

Take immediately at the onset of migraine pain — don't wait until it's severe. The sooner you take it, the more effective it is. If you experience aura, take the medication when the headache phase begins, not during the aura.

Chest/throat tightness after taking a Triptan is called 'triptan sensation', occurring in about 5% of users. It is usually unrelated to the heart and not dangerous, resolving within 15–30 minutes. However, seek immediate medical attention if you experience severe chest pain radiating to the arm/jaw or difficulty breathing.

No. Sumigran is for acute migraine treatment only, not prevention. If you experience frequent migraines (more than 4 times/month), consult your doctor about preventive medications such as Propranolol, Topiramate, or Amitriptyline.

Use with caution. Sumatriptan can be used with SSRI/SNRI but carries a risk of Serotonin Syndrome (rare). Consult your doctor first and watch for symptoms of agitation, muscle twitching, or fever after taking.

Do not exceed 300 mg/day and avoid using more than 10 days/month, as overuse may cause Medication Overuse Headache (MOH) — chronic headache caused by excessive painkiller use. If frequent use is needed, consult your doctor about preventive therapy.
