About Melodia
What is Melodia?
Melodia is a low-dose Combined Oral Contraceptive (COC) containing Drospirenone 3 mg and Ethinylestradiol 20 mcg, manufactured by Bayer.
Drospirenone is a fourth-generation progestin with two special properties: anti-androgenic (reduces male hormone effects, helping clear acne) and anti-mineralocorticoid (reduces water retention, alleviating PMS bloating).
Melodia has the exact same formulation as Yaz — Drospirenone 3 mg + EE 20 mcg in a 24+4 regimen (24 active pills + 4 placebo pills). Only the trade name differs by region.
Compared to Yasmin, which contains the same Drospirenone but with a higher EE dose (30 mcg) in a 21+7 regimen.
What is Melodia used for?
Primary indications:
- Contraception — Over 99% effective pregnancy prevention
- Acne reduction — Drospirenone's anti-androgenic effect effectively clears hormonal acne
- PMS water retention relief — Anti-mineralocorticoid effect reduces bloating and temporary weight gain before periods
Key advantages of Melodia:
- Low-dose EE 20 mcg reduces oestrogen-related side effects
- 24+4 regimen shortens the hormone-free interval, reducing headaches/mood swings during the break
How to Take Melodia / Dosage
Dosage: Each pack contains 28 tablets (24 active + 4 placebo). Take 1 tablet daily at the same time.
How to take:
- Take 24 pink active pills followed by 4 white placebo pills → start new pack immediately (no break)
- Withdrawal bleed occurs during the 4 placebo days
- Start on day 1 of your period for immediate contraceptive effect
If you miss a pill:
- Less than 12 hours late — take it as soon as you remember, continue as normal
- More than 12 hours late — take the missed pill immediately and use condoms for 7 days
Side Effects of Melodia
Common side effects:
- Nausea — usually resolves after the first 1–3 months
- Breast tenderness
- Headache
- Mood changes
- Breakthrough bleeding during initial use
Serious side effects (rare):
- DVT/PE (blood clots) — slightly increased risk
- Hyperkalemia (high potassium) — Drospirenone has spironolactone-like effects; caution in patients with kidney problems or taking potassium-raising medications
Important: Seek immediate medical attention if you experience swelling in one leg, sudden chest pain, or difficulty breathing.
Contraindications & Precautions
Do not use:
- History of DVT/PE (blood clots)
- Migraine with aura
- Smokers over age 35
- History of breast cancer
- Severe liver or kidney disease
- Pregnant or suspected pregnancy
Special precautions for Drospirenone:
- Kidney problems — Drospirenone may increase blood potassium levels
- Concurrent use of ACE inhibitors, ARBs, NSAIDs, or potassium-sparing diuretics
- Breastfeeding — consider Cerazette instead
How Does Melodia Compare to Other Contraceptives?
Melodia vs. Yaz: Identical formulation — Drospirenone 3 mg + EE 20 mcg in a 24+4 regimen. Only the trade name and distributor differ by country. Equivalent efficacy.
Melodia vs. Yasmin: Same progestin (Drospirenone) but Yasmin has higher EE (30 mcg vs 20 mcg) in a 21+7 regimen. Yasmin offers better cycle control; Melodia has fewer oestrogen side effects.
Melodia vs. Mercilon: Same EE dose (20 mcg) but Mercilon uses Desogestrel instead of Drospirenone, lacking anti-mineralocorticoid effects. Melodia is better for acne and water retention.
Melodia vs. Cerazette: Cerazette is a progestin-only pill without oestrogen. Suitable for breastfeeding or oestrogen contraindications.
How Does Melodia Work?
Melodia works through 3 main mechanisms:
- Ovulation suppression — Drospirenone + EE suppress FSH and LH
- Cervical mucus thickening — Makes sperm penetration difficult
- Endometrial thinning — Reduces implantation likelihood
Additional benefits of Drospirenone:
- Anti-androgenic: Blocks DHT binding to androgen receptors, reduces sebum production, helping clear acne
- Anti-mineralocorticoid: Spironolactone-like effect helps excrete excess water, reducing bloating and temporary weight gain before periods
The 24+4 regimen reduces the hormone-free interval from 7 to 4 days, maintaining more stable hormone levels and reducing headaches and mood swings.

Both contain Drospirenone, but Yasmin has EE 30 mcg in a 21+7 regimen, while Melodia has lower EE 20 mcg in a 24+4 regimen. Melodia has fewer oestrogen side effects and a shorter hormone-free interval.
You should take it at the same time daily, with a 12-hour grace period. If less than 12 hours late, you're still protected. Setting a phone alarm is recommended. Consistent timing helps maintain stable hormone levels and reduces breakthrough bleeding.
