About Nasonex

What is Nasonex?

Nasonex is an intranasal corticosteroid spray containing Mometasone Furoate 50 mcg per spray, manufactured by MSD/Organon.

Mometasone Furoate is a synthetic corticosteroid with high anti-inflammatory potency. It works by inhibiting the release of multiple inflammatory mediators (cytokines, leukotrienes, prostaglandins) in the nasal mucosa.

A key advantage of Mometasone is its very low systemic bioavailability (<0.1%), resulting in minimal systemic side effects. This makes it suitable for long-term use.

Nasonex is the originator brand of Mometasone Furoate nasal spray, widely used worldwide.

What is Nasonex used for?

Primary indications:

  • Allergic Rhinitis — Both perennial and seasonal: sneezing, nasal congestion, runny nose, itchy nose
  • Nasal Polyps — Reduces polyp size and relieves nasal congestion
  • Rhinosinusitis — Used alongside antibiotics for acute sinusitis

Not for: Acute nasal congestion relief (not a decongestant). Must be used regularly for 1–2 days before effects become noticeable.

How to Use Nasonex / Dosage

Dosage for allergic rhinitis:

  • Adults and children 12+ years: 2 sprays per nostril once daily (total 200 mcg/day)
  • Children 3–11 years: 1 spray per nostril once daily (total 100 mcg/day)

How to use:

  • Shake the bottle before each use
  • Blow your nose before spraying
  • Tilt your head slightly forward, insert the nozzle into the nostril pointing towards the outer wall (away from the septum)
  • Press 1–2 sprays while breathing in gently
  • Use at the same time each day, morning or evening

Important:

  • Must be used consistently every day, not just when symptomatic
  • Full effect is seen after 1–2 weeks of regular use
  • Clean the nozzle regularly

Side Effects of Nasonex

Common side effects:

  • Nosebleed (epistaxis) — most common, usually mild
  • Nasal burning or irritation
  • Sneezing after spraying
  • Headache

Serious side effects (very rare):

  • Nasal septal perforation — very rare, usually from incorrect spray technique (spraying directly at the septum)
  • Nasal or throat fungal infection (candidiasis) — uncommon
  • Glaucoma / cataracts — very rare with nasal use due to minimal systemic absorption

Important: Correct spray technique (directing the nozzle towards the outer wall, away from the septum) significantly reduces the risk of nosebleeds and septal perforation.

Contraindications & Precautions

Do not use:

  • Allergy to Mometasone Furoate or any component of the medication
  • Untreated nasal infections (e.g., tuberculosis, fungal infections)
  • Recent nasal surgery or nasal wounds — wait until healed

Use with caution:

  • Concurrent use of other corticosteroids — may increase cumulative side effects
  • History of glaucoma / cataracts — regular eye examinations recommended
  • Children — approved from age 3, but growth should be monitored
  • Pregnancy / breastfeeding — use when benefits outweigh risks as determined by a doctor

How Does Nasonex Compare to Other Allergy Medications?

Nasonex vs. oral antihistamines (Loratadine, Fexofenadine): Oral antihistamines help with sneezing, itching, and runny nose but are less effective at relieving nasal congestion. Nasonex is much better for congestion as it directly reduces inflammation. Both can be used together for maximum effect.

Nasonex vs. Fluticasone (Flixonase): Both are intranasal corticosteroids with similar efficacy. Mometasone has slightly lower systemic bioavailability than Fluticasone (<0.1% vs ~0.5%).

Nasonex vs. nasal decongestants (Oxymetazoline): Decongestants work fast (within minutes) but must not be used for more than 3–5 days due to rebound congestion. Nasonex is safe for long-term use.

How Does Nasonex Work / How Long Does It Take?

Mometasone Furoate works by suppressing inflammatory processes in the nasal mucosa:

  • Reduces inflammatory mediator release — Inhibits cytokines, leukotrienes, and prostaglandins that cause nasal mucosal swelling and irritation
  • Reduces nasal mucosal swelling — Opens airways for easier breathing
  • Reduces mucus secretion — Decreases runny nose and sneezing

Onset timeline:

  • 12–36 hours: Mild improvement in symptoms
  • 2–3 days: Noticeable reduction in congestion and sneezing
  • 1–2 weeks: Full effect — significant improvement in all symptoms

Must be used consistently every day as prescribed, even after symptoms improve, to maintain long-term inflammation control.

Frequently Asked Questions

Not immediately. Nasonex is not a fast-acting decongestant. It takes 1–2 days of regular use to start noticing improvement, with full effect in 1–2 weeks. For immediate relief, a short course of decongestant nasal spray (max 3–5 days) can be used alongside.

Yes. Using Nasonex alongside oral antihistamines (such as Loratadine, Fexofenadine, Cetirizine) is more effective than either alone, especially for severe symptoms. Nasonex addresses congestion and inflammation while antihistamines reduce sneezing, itching, and runny nose.

Mometasone in Nasonex has less than 0.1% systemic absorption, making it very different from oral or injectable steroids. Experts confirm it is safe for long-term use as indicated. However, regular follow-up with your doctor is recommended.

Yes. Nasonex is approved for children aged 3 and above. Children 3–11 years use 1 spray per nostril once daily; children 12+ use the same adult dose. Parents should supervise spray technique and monitor growth.

Mild nosebleeds are a common side effect and usually don't require stopping. Try adjusting your spray technique — direct the nozzle towards the outer wall (opposite ear), not at the septum. If bleeding is heavy or frequent, consult your pharmacist or doctor.

Yes. Nasonex reduces nasal inflammation triggered by all types of allergens, including PM2.5 dust, dust mites, pollen, pet dander, and mould. However, allergen avoidance (masks, air purifiers) should also be practised alongside medication.
