Yaz

Yaz is a combined oral contraceptive (COC) containing two active ingredients: Drospirenone 3 mg (a 4th-generation progestin) and Ethinylestradiol 0.02 mg (estrogen).

Do not use:

• History of venous or arterial thromboembolism, or high risk factors
• Severe liver disease, liver tumors, or liver cancer
• Breast cancer or hormone-sensitive cancers
• Smokers over age 35 — significantly increases cardiovascular and stroke risk
• Migraine with aura
• Pregnancy or breastfeeding
• Renal insufficiency or adrenal gland dysfunction (Drospirenone may increase potassium levels)

Use with caution:

• High blood pressure — monitor regularly
• Family history of blood clots
• Obesity or prolonged immobilization

Yaz vs. Yasmin: Both contain Drospirenone. Yaz has lower estrogen (0.02 mg vs. 0.03 mg) and uses a 24/4 regimen, while Yasmin uses 21/7. Yaz is FDA-approved for acne and PMDD; Yasmin is approved for contraception only.

Yaz vs. Levonorgestrel-based pills (2nd-generation progestin): Drospirenone in Yaz has anti-androgenic and anti-water retention properties, resulting in fewer side effects related to acne, weight gain, and bloating. However, Drospirenone-containing pills may carry a slightly higher VTE risk than 2nd-generation pills, though the absolute risk remains very low in women without risk factors.

Medications requiring caution:

• Lamotrigine (anti-epileptic) — Yaz may reduce Lamotrigine blood levels
• Potassium-increasing drugs: ACE inhibitors, ARBs, Spironolactone, potassium-sparing diuretics, chronic NSAIDs — Drospirenone may raise potassium levels
• Tranexamic acid — contraindicated, increases thrombotic risk

Most common antibiotics (e.g., Amoxicillin, Azithromycin) do not affect Yaz, except Rifampicin. Always inform your doctor or pharmacist that you are taking oral contraceptives.
